Problem: Drug mix-ups caused by a failure to confirm the product name prior to administration. 

Solution: We created this low-tech solution for high-alert drug vials. Flexible PVC rings with bold warnings fit around the neck of vials and encourage workers to stop and double check the drug name prior to administration.
