Wrong-route injections can have devastating and long-lasting results, but one piece of tape can help minimize the risk for mistakes.
The ISMP (Institute for Safe Medication Practices) has reported multiple cases involving accidental injections of tranexamic acid into the spine, all because of container mix-ups. According to the report, bupivacaine, ropivacaine and tranexamic acid are packaged in vials that may have the same blue cap and can easily be confused.
A single piece of warning tape across the tops of vials containing tranexamic acid can help prevent similar situations among drugs that look similar. Tape is perforated for easy removal and unlike paper-backed labels, will not unravel.
